Assertive Community Treatment (ACT)
Assertive Community Treatment (ACT) is a community based service available to individuals suffering from severe and persistent mental illness who need more intensive levels of support to manage symptoms of their illness. The ACT Team is a multidisciplinary group of professionals representing the fields of psychiatry, social work, nursing, counseling, addictive disease, vocational rehabilitation, case management, and peer support. ACT services are individually centered, recovery oriented, and highly intensive. In addition to providing services that are individually developed to reflect preferences by the recipient, services will occur, whenever possible, in an individual's natural environment. ACT services are available to individuals 24/7 and are designed to provide interventions that reduce the frequent utilization of acute care services.
